Contents:

          Metformin HCl 500 mg

Indications:

          Type 2 DM in adults particularly in overwt patients when dietary management & exercise alone does not result in adequate glycaemic control, as monotherapy or in combination w/ other oral antidiabetics or w/ insulin in adult & as monotherapy or in combination w/ insulin in childn from 10 yr & adolescent.

Contraindications:

          Diabetic ketoacidosis & pre-coma; renal failure/dysfunction (CrCl <60 mL/min); dehydration, severe infection, shock, intravascular administration of iodinated contrast agents; cardiac or resp failure, recent MI; hepatic insufficiency, acute alcohol intoxication, alcoholism. Childn.

Special Precautions:

          Lactic acidosis may occur in diabetic patients w/ significant renal failure; poorly controlled diabetes ketosis, prolonged fasting, excessive alcohol intake, hepatic insufficiency & hypoxia-associated conditions. Monitor CrCl &/or serum creatinine levels & perform regular laboratory tests for diabetes. Discontinue therapy 48 hr before surgery & should not be resumed earlier than 48 hr. Energy-restricted diet should be continued in overwt patients. Pregnancy & lactation. Elderly.

Dosage:

          Monotherapy & in combination w/ other oral antidiabetics or insulin. Usual starting dose: Adult 500 or 850 mg bd-tds, adjust after 10-15 days. Max dose: 3 g/day. Glucophage XR 500-mg tab Monotherapy & in combination w/ other oral antidiabetics 500 mg once daily, may be adjsuted after 10-15 days. Max: 2000 mg daily. If glycaemic control is not achievable: 1000 mg bd, may be switched to standard metformin tab up to a max of 3000 mg daily. Glucophage XR 750-mg tab Dose should be equivalent to the daily dose of metformin tab (extended or immediate release) up to a max of 1500 mg, may be adjusted after 10-15 days. Combination w/ insulin Usual starting dose: 500 mg once daily. Patients already treated w/ metformin & insulin in combination therapy Dose should be equivalent to the daily dose of metformin tab up to a max of 1500 mg.
