Indications:
Treatment of HTN; heart failure (NYHA class II-IV) in patients receiving standard therapy eg diuretics, digitalis & either ACE
inhibitors or β-blockers but not both; presence of all these standard
therapies is not mandatory. To improve survival following recent MI in
clinically stable patients w/ signs, symptoms or radiological evidence
of left ventricular failure &/or w/ left ventricular systolic dysfunction.
Contraindications:
Severe hepatic impairment, cirrhosis, biliary
obstruction. Severe renal impairment (CrCl <10 mL/min) & patients
on dialysis. Pregnancy.
Dosage:
HTN 80 mg once daily, may be increased to 160 mg daily or a diuretic may be added. Max: 320 mg. Heart failure
Initially 40 mg bd. Up titration to 80 & 160 mg bd should be done
to the highest dose tolerated by the patient. Max: 320 mg. Post-MI Initially 20 mg bd increased to 40, 80 & 160 mg bd over the next few wk. May be initiated as early as 12 hr post-MI. Patients w/ Na- &/or vol-depletion Initially 40 mg.

Updating
Severe Na- or vol-depletion. Patients w/ renal
artery stenosis; preexisting renal dysfunction or severe cardiac
insufficiency. Patients w/ heart failure or post-MI; renal function
should be assessed. Avoid concomitant use w/ ACE inhibitor &
β-blocker in patients w/ heart failure. May impair ability to drive or
operate machinery. Lactation. Childn & adolescents <18 yr.
