         Isotretinoin

Indications:

          Severe forms of therapy-resistant nodulo-cystic acne particularly cystic acne & acne conglobata esp when the lesions involve the trunk.

Contraindications:

          Hepatic or renal insufficiency, hypervitaminosis A, excessive elevation of blood lipid values, pregnancy, lactation.

Special Precautions:

          Monitor liver function, visual problem & signs of depression. Presence of gyneco-endocrinological problems. Avoid dermabrasion, wax epilation & prolonged exposure to sunlight. Frequent checks of serum lipids &/or blood glucose in patients w/ diabetes, obesity, alcoholism or disorders of lipid metabolism.

Dosage:

         Initially 0.5 mg/kg daily. Maintenance dose: 0.1-1 mg/kg/day or up to 2 mg/kg in patients w/ very severe disease or w/ truncal acne. Duration: 16 wk.
