Carbamazepine.

Indications:

Epilepsy. Acute mania & maintenance treatment of bipolar-affective disorders to prevent or attenuate recurrence. Alcohol w/drawal syndrome. Idiopathic trigeminal neuralgia & trigeminal neuralgia due to multiple sclerosis (either typical or atypical). Idiopathic glossopharyngeal neuralgia. Painful diabetic neuropathy. Diabetes insipidus centralis. Polyuria & polydipsia of neurohormonal origin.

Contraindications:

AV block. History of bone marrow depression & hepatic porphyrias. Concomitant MAOIs.

Special Precautions:

Initial & periodic complete blood counts, liver function tests & urinalysis. Monitor plasma levels. History of cardiac, hepatic or renal damage, adverse haematological reactions, interrupted courses of Tegretol therapy. Mixed seizures, increased intraocular pressure. Activation of latent psychosis, confusion, agitation, suicidal ideation & behaviour. Abrupt w/drawal of treatment. Breakthrough bleeding in women using hormonal contraceptives. May impair ability to drive or operate machinery. Pregnancy & lactation. Elderly.

Dosage:

Epilepsy Adult Initially 100-200 mg once-bd, increased gradually to 400 mg bd-tds. Childn >4 yr Initially 100 mg/day, increased at wkly intervals by 100 mg; ≤4 yr Initially 20-60 mg/day, increasing by 20-60 mg every 2nd day. Maintenance dose: 10-20 mg/kg/day in divided doses. Trigeminal neuralgia Initially 200-400 mg/day, increased gradually until freedom from pain is achieved (normally 200 mg tds-qds). Then reduce dosage to lowest possible maintenance level. Elderly Initially 100 mg bd. Alcohol w/drawal syndrome 200 mg tds, increased in severe cases to 400 mg tds for 1st few days. Diabetes insipidus centralis Adult 200 mg bd-tds. Painful diabetic neuropathy 200 mg bd-qds. Acute mania & maintenance treatment of bipolar-affective disorders Dosage range: 400-1600 mg daily. Usual dosage: 400-600 mg daily in 2-3 divided doses.
