Indications:
Adjunctive therapy in the treatment of partial seizures w/ & w/o secondary generalization in adult & childn ≥3 yr. Treatment of neuropathic pain including diabetic pain, postherpetic & trigeminal neuralgia.
Contraindications:
Hypersensitivity to any of the ingredients
Special Precautions:
Avoid abrupt w/drawal of therapy. Not effective in the treatment of absence seizures. Monitor for signs of suicidal ideation & behaviours. Neuropsychiatric adverse events in childn. Monitor for signs & symptoms of severe, life-threatening, systemic hypersensitivity reactions eg drug rash w/ eosinophilia & systemic symptoms (DRESS). May impair ability to drive or operate machinery. Pregnancy & lactation.
Dosage:
Epilepsy Adult & childn >12 yr Initially 300 mg tds on day 1 or by titrating dose as: 300 mg once on day 1, 300 mg bd on day 2 & 300 mg tds on day 3. Thereafter, dose can be increased in 3 equally divided doses up to 3600 mg daily. Childn 3-12 yr Initially 10-15 mg/kg/day in 3 equally divided doses. The effective dose reached over a titration period of 3 days; ≥5 yr Effective dose: 25-35 mg/kg/day; 3 to <5 yr 40 mg/kg/day in 3 equally divided doses up to 50 mg/kg/day. Max dose interval: 12 hr. Neuropathic pain Adult Initially 300 mg once on day 1, 300 mg bd on day 2 & 300 mg tds on day 3. Max: 3600 mg daily. Patient w/ neuropathic pain or epilepsy w/ impaired renal function CrCl ≥80 mL/min 900-3600 mg/day; 50-79 mL/min 600-1800 mg/day; 30-49 mL/min 300-900 mg/day; 15-29 mL/min 150-600 mg/day (150 mg to be administered as 300 mg every other day); <15 mL/min 150-300 mg daily (150 mg to be administered as 300 mg every other day). Hemodialysis Loading dose: 300-400 mg, then 200-300 mg following each 4 hr of hemodialysis.
