Bisoprolol hemifumarate
Indications:
HTN, CHD (angina pectoris); stable chronic heart failure w/ reduced systolic ventricular function in addition to ACE inhibitors & diuretics, & optionally cardiac glycosides.
Contraindications:
Acute heart failure or during episodes of heart failure decompensation requiring IV inotropic therapy; cardiogenic shock, 2nd or 3rd degree AV block (w/o pacemaker), sick sinus syndrome, SA block, symptomatic bradycardia or hypotension; severe bronchial asthma or COPD; late stages of peripheral arterial occlusive disease & Raynaud's syndrome; untreated phaeochromocytoma, metabolic acidosis. Childn.
Special Precautions:
Ischaemic heart disease, DM, strict fasting, ongoing desensitisation therapy, 1st degree AV block, Prinzmetal's angina, peripheral arterial occlusive disease; IDDM, severe renal or hepatic impairment, restrictive cardiomyopathy, congenital heart disease, haemodynamically significant organic valvular disease, MI w/in 3 mth; bronchial asthma or other COPD; existing or history of psoriasis, phaeochromocytoma. May mask thyrotoxicosis symptoms. Patients on general anaesth therapy. May impair ability to drive or operate machinery. Pregnancy & lactation.
Dosage:
HTN or angina pectoris 5 mg once daily, may be increased to 10 mg once daily. Stable chronic heart failure Patient must be treated w/ an ACE inhibitor, a diuretic & optionally cardiac glycosides, prior to the administration of bisoprolol. Titrate gradually as follows: 1.25 mg once daily for 1 wk then 2.5 mg once daily for a further wk then 3.75 mg once daily for a further wk then 5 mg once daily for the 4 following wk then 7.5 mg once daily for the 4 following wk and then 10 mg once daily for the maintenance therapy. Max: 10 mg once daily. Severe renal insufficiency CrCl <20 mL/min & liver function disorders Max daily dose: 10 mg.
